Health Insurance

If you have health insurance through your employer, or have an individual policy, you should contact that insurance carrier to determine if the claim for this account was already submitted. You are the subscriber, and they have representatives who should be able to answer your questions, and give you a status on why payment has not yet been made.

* Note: Each carrier has their own filing limits, so please be sure to investigate any discrepancies with your carrier promptly.

In the event your carrier has no record of this claim, we will assist you by submitting a claim on your behalf. You will need to contact our office to give us the necessary information. This may be done, by faxing us a copy of your insurace card, both front and back or you can call and ask for a collection representative. We will need your name, address and phone number, and the subscriber ID#, Insurance carrier's name, address, and phone number. Please have all information available when you call.

Auto Insurance

If you were involved in an automobile accident, your treatment may be covered under the No-Fault provisions in the Insurance law. In the state of Florida, each driver should submit claims to their own automobile insurance carrier for payment of injury related claims, regardless of fault.

* Please Note: The insured should report the accident to his/her insurance carrier within 30 days. Potential claimants must apply for No-Fault benefits before any claims will be considered for payment.

If you were involved in an automobile accident during the course of employment, you should immediately file a Workers' Compensation claim with your employer. For more information regarding Workers' Compensation, click on the button at left.

If you believe you are covered under No-Fault insurance, and you have verified this coverage, as well as verified that the claim has not yet been submitted to the insurance carrier, we will assist you by filing the claim on your behalf.

Workers Compensation

Worker's Compensation covers employees injured during the course of employment. If you were injured on the job, you must report the injury to your employer. Employers are required by law to be insured through an insurance company or self-insured and MUST display proof of insurance at the workplace. Once your accident is reported you will receive a claim number assigned to your injury, which must be given to your health care providers in order for your treatment to be paid. Please see your employer regarding benefits.

If you believe your medical services should have been covered under Workers' Compensation, Please contact the insurance carrier and determine if this claim was filed, why it has not been paid at this time, and get a status on when a check will be sent to the provider. If your employer has denied your workman's comp claim, you can contact the Florida Employee Assistance line at 1-800-342-1741.

In the event the Workers' Compensation insurance carrier has no record of this claim, we will assist you by re-submitting the claim on your behalf. Please contact our office and ask for an collection representative. We will need your name, address, phone number, Insurance company's name, address, phone number, date of the accident, and claim number. Please have this information available when you call.

Medicare

If you are 65 or older, have end stage renal failure, or are permanently disabled, you probably have Medicare Benefits. Please contact Medicare to determine if this claim was filed, why it has not been paid at this time, and get a status on when a check will be sent to the provider.

For Your Information:

Medicare Part A provides benefits for In-patient hospitalizations. Medicare part B provides benefits for Out-patient services such as; ambulatory surgery, pre-surgical testing, physician services, and diagnostic testing.

Your Medicare card will indicate if you have Part A or Part B, or both, and their effective dates. If you are 65 or over, but you or your spouse are still actively employed, your employer group health plan may be the primary carrier for medical claims. In this case you would need to be sure that the claim was submitted to the primary carrier prior to Medicare.

If the Medicare has no record of this claim, we will assist you by communicating this information to the hospital, or doctor's office, and requesting that the claim is re-submitted. Please call and ask for an insurance billing representative. We will need your name, address, phone number, and the Medicare #.

Medicaid

If you have Medicaid coverage you need to contact your caseworker, or the Medicaid office, to determine if the claim for this account was already submitted. They have representatives who should be able to answer your questions, and give you a status on why payment has not yet been made.

* Note: Medicaid does have a filing limit, so please be sure to investigate any discrepancies with your carrier promptly. In the event they have no record of this claim, we will assist you by submitting a claim on your behalf. You will need to contact our office to give us the necessary information.
